Obtaining Your Medical Records

The Health Information Services (HIS) department at Beaufort Memorial Hospital fulfills requests for information for all inpatient, outpatient and clinical services.

Electronic Request for Records

Beaufort Memorial now offers an Online Patient Record Request tool for the following individuals to request patient records:

  • Patients requesting their own records
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The Online tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. If the patient is deceased, please make sure to attach a copy of the death certificate. There is no additional charge to use this service beyond fees outlined below.

Please note that fees are charged for copies of medical records that are not requested by healthcare providers for direct patient care. To reduce costs incurred, patients should consider requesting specific information rather than a complete record. Fees for personal requests of medical records by patients are as follows:

  • $0.90 labor fee
  • $0.05 per page for supplies (paper/toner)
  • $6.50 for CDs/delivery in electronic format (as electronic medical record (EMR) system allows)
  • $50 flat fee for Pathology slides

For other types of requests, such as those from attorneys or insurance providers, additional fees may apply. Learn more.

Payment must be made before records are sent. Records requests are usually processed within seven to ten business days (South Carolina State Statute, Sections 44-115-80 and 44-7-325).

The HIS staff is happy to assist anyone unable to use the online patient record request tool with obtaining medical records upon receipt of a valid authorization. You can mail, fax or personally deliver your authorization to release health information.
