Hospital Discharge & Ongoing Care

Beaufort Memorial physicians and staff are dedicated to your ongoing health after leaving the hospital. Many services are available to help you continue to heal after hospital discharge. Following your provider’s discharge plan will go a long way to ensure your recovery is the best it can be.

Hospital Discharge

Once your physician has determined you are well enough to leave the hospital, they will submit a discharge order. There are a few items you can do to make your discharge easier:

  1. When possible, send flowers and/or gifts as well as extra clothing or personal items home the day before.
  2. Ensure you have transportation arranged for your time of discharge.
  3. Discuss billing arrangements with a financial counselor to help ensure everything is correct before leaving.

Bedside Prescription Delivery

pharmacy tech delivering a prescription to a patientYou can opt to have all of your prescriptions and over-the-counter medication orders filled by the Publix Pharmacy and delivered right to your room, so you’ll have everything you need without having to stop on the way home.

The delivery service is free of charge and available Monday through Friday, 9a.m. to 6 p.m. Let your nurse know if you are interested in bedside delivery of your medications. Please have your prescription card and co-pay ready to pay for your medication at the time of delivery. Cash, credit and debit (including FSA) are accepted.

Continuing Care

Inpatient Rehabilitation

In some situations, you may need help building the skills you need to go home, including self-care skills, functional mobility, bowel and bladder management and wound care. The staff in our Acute Inpatient Rehabilitation Unit will work with you and your family each day in one-on-one therapy, group therapy, community re-entry activities, patient and family education and goal-directed activities.

Outpatient Rehabilitation

To assist with restoring your physical strength, cognition, mobility, independence and self-sufficiency, Beaufort Memorial’s outpatient rehabilitation services offer physical therapy, occupational therapy and speech therapy for a wide variety of conditions affecting children and adults. We provide pediatric, adult and geriatric services at locations in Beaufort and Bluffton.

Home Health Care

If you will need equipment or skilled care at home, Beaufort Memorial’s Care Coordination team will work with you to make arrangements for what you need before leaving the hospital.

Cardiac and Pulmonary Rehabilitation

To help you get back to a normal life after a cardiac event, Beaufort Memorial offers a medically supervised exercise, nutrition and education program. You’ll find friendly support and guidance from our medical professionals in a designated area of the LifeFit Wellness Center.

LifeFit Wellness Services also has an experienced team of pulmonary rehabilitation specialists who will work with you and your physician to improve your breathing through regular exercise and symptom management.

Skilled Nursing Facilities

If you are ready to leave the hospital, but not yet strong enough to return home, Beaufort Memorial’s Care Coordination team can discuss your options for short-term therapy in a skilled nursing facility to help you with your transition back home.

Palliative Care — Supportive Care for Complex Illnesses

If you have a serious illness, Beaufort Memorial’s Care Coordination team will help connect you with palliative/supportive/comfort care services to help you manage both your stress and symptoms. Beaufort Memorial’s Care Coordination team will help connect you with services to address emotional and physical discomfort during treatment and help you and your family understand how to maintain the highest quality of life possible.

Routine Healthcare

One of the best ways to stay out of the hospital is to develop a habit of routine checkups. Building a relationship with your primary care provider can go a long way to identify problem areas early and take steps to prevent further damage — sometimes even reversing a condition.
